Focal Community Services Support Coordination team provides person centered, quality services that provide goal orientated supports to our participants . We work alongside our participants and listen to their needs to implement relevant services to meet their specific goals. Our team of dedicated support coordinators have capacity to take on new participants and are eager to provide assistance to guide you through your NDIS plan.
