Outcare - Community Inclusion - Bayswater

Community Inclusion supports participants to achieve their aspirations and build their capability. We support participants for the long-term, setting goals together to reintegrate and improve connections and with community and family. We build capacity through routine support with everyday tasks, including building & maintaining healthy relationships or attending justice appointments. Our goal is to maximise our clients interdepencence with community and thereby reduce recidivism.
